| The Healthy 401(k) Investment Program is designed to encourage employees to live healthy lives and participate in the many health programs and benefits offered by Southern Farm Bureau Life Insurance Company. It is our goal as the WellnessWorks! program to create a healthy culture here at SFBLI and encourage our employees to be the best health consumers that they can be. During the Healthy 401(k) Incentive Program, we hope to share more information on how living a healthy lifestyle can be fun and rewarding, and also shed a little light on investing in general.
The program is a simple step by step process which rewards employees with WellnessBucks! that can be used in the SFBLI company store or applied at the end of the program to some specially picked wellness items. For employees who complete all four steps throughout the Healthy 401(k) Incentive Program's process, we will have two drawings for 2 nights stay for 2 people in Destin Florida!
How does it work?
Step One is to register for the program by contacting Matt Ginn and completing an online survey with a few questions and behavior goals.
Step Two is to schedule and complete your Healthy You! Preventive exam offered by the SFBLI health plan which is managed by Blue Cross Blue Shield MS. Take your Guide to a Healthy You! with you and talk with your doctor about the tests, your health goals, and other prevention questions you may have at the time of the visit.
Step Three is to make deposits in both small cap and large cap fund strategies and record them on your Healthy 401(k) Planner. The strategies are specifically designed to help you optimize the employee benefits, wellness program, onsite resources, and community options. For a listing of strategy explanations, click here.
* As you complete level-one activities, you will be eligible to receive WellnessBucks!
*You must do a minimum of ten level-one activities using all four strategies to be eligible for trip incentive.
*You must do a minimum of three level-two activities out of at least two different strategies to be eligible for trip incentive.

There will be quarterly reviews of these options so bring your statement to Matt Ginn and he will give you the WellnessBucks! that you earned.
Step Four is to turn in your Healthy 401(k) Planner to Matt Ginn with all options checked off and signed at the bottom to ensure that you completed all of the actions checked off. You will be asked to complete another online survey to give feedback on what you thought of the program and if the program helped you.
Trip Incentive Details - SFBLI WellnessWorks! will have a drawing for eligible employees at the end of the Healthy 401(k) Incentive Program for two trips for two to Destin Florida. The trip will consist of a two nights stay at a beachside condo and $250 spending cash. The drawing will be held in October 2009 and the trips will be good for November 2009 through October 2010.
Participants should have fun, encourage others along the way, and gain the many dividends of being a healthy and engaged employee.
